FMG Nurse
A follow-up service with a clinical nurse (FMG) is offered to patients who are registered with a family doctor at the Fontainebleau FMG after a referral from your doctor. This service was created to improve follow-ups with patients suffering from certain chronic illnesses:
- Diabetes
- Arterial hypertension
- Assessment and follow-ups for issues related to cognitive abilities
- Follow-up and adjustment of anticoagulation therapies (INR)
- Other follow-ups requested by the doctor
- Depression
- ADHD: Diagnosis and follow-up
- Hypercholesterolemia
- Pap test
- STD tests
The doctor and the clinical nurse will work in collaboration to provide the appropriate medical follow-ups for you. This follow-up service will allow doctors to get the necessary information to optimize your treatment and make accessing services easier for you.
If you have already been evaluated by one of our clinical nurses for a chronic problem (ex.: diabetes, hypertension, etc.), you can directly contact that nurse by calling the clinic. She will then evaluate you and refer you to a doctor if necessary.
